Key Responsibilities
Oversee all rehabilitation department operations and ensure delivery of high-quality therapy services
Supervise, mentor, and lead PTs, OTs, SLPs, therapy assistants, and support staff
Manage staffing levels and coverage to meet resident needs
Oversee individualized care plans and conduct quality assurance reviews
Ensure accurate, timely documentation compliant with Medicare, Medicaid, and other payer guidelines
Lead Utilization Review (UR) meetings and monitor treatment effectiveness
Participate in care plan meetings and discharge planning with the interdisciplinary team
Manage departmental budget and expenses while maintaining excellent outcomes
Identify opportunities for new programs and services
Requirements
Graduate of an accredited PT, PTA, OT, or COTA program
Active, unencumbered Minnesota license (PT, PTA, OT, or COTA)
CPR certification
3–5 years of clinical experience in a SNF or similar setting
Minimum 2 years of Rehab Director | management experience
Proficiency with therapy documentation systems (e.g., PointClickCare, Casamba)
Working knowledge of federal and state regulations, including Medicare and Medicaid
