Position Overview
An established community health center in Connecticut is seeking an experienced Advanced Practice Registered Nurse or Physician Assistant to provide comprehensive, patient-centered care to a diverse community. The successful candidate will combine sound clinical judgment with effective collaboration, culturally responsive care and timely documentation.
The organization provides an integrated medical home that includes medical, behavioral health, dental and pharmacy services. Its clinical teams focus on accessible, dignified care for individuals and families, including patients with complex healthcare needs. Qualifications and Clinical Expectations
• Graduate of an accredited school of nursing with successful completion of an approved Nurse Practitioner program, or graduate of an accredited Physician Assistant program.
• Valid Connecticut professional license.
• Current DEA registration and Connecticut controlled-substance registration, or eligibility to obtain both.
• Strong knowledge of advanced nursing practice or physician assistant practice, supported by a record of high-quality, comprehensive clinical care.
• Ability to obtain medical histories, assess clinical conditions and interpret findings accurately.
• Ability to provide culturally competent care to a diverse patient population within a collaborative environment.
• Effective oral and written communication with patients, colleagues and other stakeholders.
• Ability to maintain timely, thorough and accurate clinical documentation.
• Sound evidence-based clinical decision-making and compliance with departmental and organizational quality measures.
• Comfort using electronic health records and related computer systems.
• Willingness to perform other duties consistent with the position as assigned.
Preferred Experience
Candidates with at least eight years of experience in an ambulatory-care or urgent-care setting are preferred. Experience serving underserved populations or working in a community health environment is highly valued.
