Patient Care Manager - Newport Beach Schedule: Full-time | Monday – Friday | 8 a.m. to 5 p.m. As Patient Care Manager, you're the link between exceptional patient care and a thriving clinical team. You ensure every patient receives compassionate, high-quality home health care — and you make that possible by guiding and empowering the clinicians who deliver it. Under your leadership, staff grow, collaborate, and do their best work, and patients feel the difference. You'll coordinate services thoughtfully, communicate clearly, and uphold professional and regulatory standards, building a culture of excellence, safety, and trust. In this role, your impact is twofold: better outcomes for patients, and a stronger, more supported team of professionals who serve them. Patient Care Manager – Key Responsibilities Lead clinical operations: Supervise clinical staff and oversee patient care to ensure services meet professional standards, agency policies, and legal and regulatory requirements. Coordinate patient care: Plan, organize, and monitor care across each patient's episode, ensuring services are delivered consistently and on time. Keep everyone connected: Maintain clear, effective communication with patients, caregivers, referral sources, and field and office staff. Drive quality outcomes: Meet departmental goals and take an active role in quality improvement initiatives. Champion compliance and safety: Complete required training, report accurately, support audits and investigations, and promote safety protocols across the team. Develop your team: Coach clinicians, encourage continuing education, and create opportunities for staff to grow. Set the standard: Model professionalism, service excellence, and organizational values every day. Patient Care Manager Qualifications Registered nurse with current licensure to practice nursing in the practicing state. Previous experience in home care setting with two years management or supervisory experience, preferred. Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
