We are looking for a motivated Physician Assistant or Nurse Practitioner to join our integrated medical practice in full time in Snowflake, AZ. We are looking for a provider who can join us providing direct patient care along with our dedicated healthcare team. Our ideal PA|NP is holistically minded, comfortable working within an integrated setting, collaborating with a knowledgeable and well-respected team of professionals, to offer a comprehensive approach to patients living with acute|chronic health issues, as well as health and wellness goals.
Duties:
• Patient exam|evaluation- medical, orthopedic, and functional examinations (medical history, physical exam, review pertinent med records|imaging)
• Order and review tests|labs
• Diagnosis and treatment plan formulation
• Patient education as to diagnosis, treatment options, rehab|exercises and various modalities
• Manage patient care- common services include allergy testing| immunotherapy, labs, prescriptions, and DME|bracing
• Treatments- trigger point|myotendinous injections, joint injections, Regenerative|PRP injections, initiate IVs, and supervising our diabetes program
• Documentation and progress notes
• Collaboration with the multidisciplinary team for comprehensive patient care
• Monitor patient progress by performing progress exams and documenting any changes to provider orders based upon patient health goals.
Requirements:
• Graduation from an accredited Physician Assistant or Nurse Practitioner program
• PA|NP license in AZ
• Orthopedic, Physical Med and Rehab, Neuro, Diabetes management or Pain management background is a plus (we will train in any|all areas needed for the right candidate)
• DEA number
• 2 years clinical experience as a PA|NP preferred
Compensation:
• $115k- $125k per year, plus bonus potential (DOE)
Schedule:
• M, T, Th, 8:00am - 5:30pm; Wed and Fri, 8:00am - 2:00pm.
