Job/Practice Description:
- Full-time, 40 hours/week
- APP will support a busy Internal Medicine team seeing adult/geriatric patients
- Nurse Practitioner will provide the co-management of chronic illnesses and conduct annual wellness visits as needed
- The provider will join a busy primary care clinic and should be comfortable and confident caring for complex and co-morbid patients
Background Profile:
- Board Certified Family Nurse Practitioner, Adult Geriatric Nurse Practitioner, or Physician Assistant
- 1 + years of experience working in primary care as an Advanced Practice Provider
